Q. How do I receive a referral for physical/occupational/speech therapy?
A. Please consult with your primary care physician to see if you would benefit from therapy. In order to begin receiving therapy from one of our skilled therapists, you must first consult with your doctor to receive a referral for therapy. After a referral is given to you by your doctor, you may contact us at (317) 542-7680 to schedule an appointment.

Q. How long will my first appointment be?
A. Please allow 60 to 90 minutes for your first appointment. Subsequent appointments vary from 45 to 75 minutes.

Q. What should I wear to physical/occupational therapy?
A. The therapist will need to expose and examine the body part (knee, shoulder, back, etc.) you are being seen for. Please wear appropriate clothing that will allow this. For all visits, wear comfortable, loose fitting clothing suitable for exercise.

Q. Will I see the same therapist every visit?
A. You will be scheduled with the same therapist for every visit to ensure consistency of care throughout your course of treatment.

Q. May I bring my spouse/caregiver/family member with me to therapy?
A. You may bring one person with you to your initial visit. For subsequent visits we request that patients only be present at treatment sessions to protect the privacy of our other patients.

Q. What do I need to bring with me to my first visit?
A. Please bring your primary and secondary (if applicable) insurance card(s) so we can verify your physical/occupational/speech therapy benefits.

Q. What insurances are accepted by Premiere Rehab, LLC?
A. Premiere Rehab, LLC accepts several insurance plans and files all insurance claims. You may call (317) 542-7680 or e-mail us to confirm if your insurance is currently accepted.

Q. Will I have a co-pay for physical/occupational/speech therapy visits?
A. Co-pays are determined by your insurance. Premiere Rehab will verify your benefits and inform you if a co-pay is required. If your insurance does require a co-pay, we ask that you pay the full amount due at each visit.

Q. Are there any forms that I will need to complete prior to my first visit?
A. In order to save time during your first visit, we ask that you print off, read and complete the patient information forms. If you have difficulty printing off the form, please call or e-mail us and we will be happy to send you the forms in the mail.
