Please visit with your primary care physician to make sure you qualify and could benefit from Physical, Occupational, or Speech Therapy. Your physician will give you a Prescription or Referral for therapy. Once you have been referred, you can either bring in your referral to our office, or have your physician fax it to us at (317) 542-7682.
